Case Management

We proudly provide case management services to children and adults with intellectual, developmental, and other disabilities across Archuleta, Dolores, La Plata, Montezuma, and San Juan Counties. Case Management is the critical first step in accessing support services. Each individual is paired with a dedicated Case Manager who offers personalized care coordination, helps determine eligibility for waiver or state-funded programs, and supports goal setting based on your unique strengths and needs. Your Case Manager will guide you through the process of selecting the most appropriate services and connect you with qualified local providers. Even after services are in place, your Case Manager will continue to check in and ensure those supports remain aligned with your evolving needs.
